Replace the original sparse README.md with a structured reference that answers the immediate questions a new or returning contributor has: how to file a ticket, how to find work, what the labels mean, where to talk.

The previous README described the repo as "mostly intended for tracking" and said it "should remain empty." The new version treats the tracker as the active tool it is becoming.
