This commit transforms the simple Markdown template used for new issues into a more beautiful, elegant issue form. It also provides better validation and user experience for people filling out the new issues. It will hopefully invite more structured input of information, e.g., new Council tickets, so we can respond more quickly and to the satisfaction of the person who reported the ticket.
