 📝 docs(coordinators): Replace embedded template with Forge issue form 
The Coordinators Handbook embedded a full Markdown issue template as a
code block for proposing new mentorship programs. This template now
lives as a proper Forgejo YAML issue form in the tickets repo
(mentored-projects/tickets#180), so the docs page replaces the 70+
line code block with a short paragraph pointing to the issue template.

Also updates the project proposal URL from the old GitLab.com Mentoring
Working Group Home repo to the Forge tickets repository.
